Cornell University Health Care Survey
Greetings from Janet Corson-Rikert, MD
Director of University Health Services at Gannett Health Center
SURVEY OVERVIEW
Cornell University is conducting an extensive study of health care and insurance issues for our students. This research effort, which includes this survey, will help us to evaluate the operation and funding of Gannett Health Center. It also will help us determine the future direction of the student health insurance program. I really appreciate your help in this effort.
We expect the survey will take approximately 5 to 10 minutes to complete.
SURVEY INSTRUCTIONS
After you finish reading this page, click on the "I'm willing to participate in the survey" button below to access the survey.
The next page is the "Survey Entry Form." Type the last six digits of your social security number in the box.
If you are an international student, please use the number that Cornell assigned for you to use when you are asked for a social security number. (It looks something like this: 999-34-5678.) You will type in the six digits after 999.
If you cannot access the survey using the last six digits of your Social Security number, please contact Rosemary at rtl1@cornell.edu or 255-3564.
If you wish to preview the survey before taking it, type in the bogus identification number: 123456
Click on the "Give this a try" button to begin the survey.
The rest of the survey is self-explanatory. (You will not need to remember these instructions.)
When you click on the "submit" button at the bottom of each page, the survey software will call your attention to any questions you did not answer. You may then use the "back" button on your web browser to answer the question. If you inadvertently answer a question incorrectly, there is a button at the bottom of each page of the survey that allows you to clear all answers on that page.
Your responses to this survey are completely confidential. We have worked carefully to assure the confidentiality of the survey data. The partial use of your social security number will not be associated in any data file with your survey answers; we will use it only to track who has completed a survey. Also, the survey is operated on secure server.
Thank you for participating in this important research effort. Best wishes for good health and success in your academic endeavors this year.
Janet Corson-Rikert, MD
